Are you a New Patient?

If you are a new patient to our practice, it is important that we obtain all pertinent information regarding your past medical and or surgical history.  We ask that you go to our forms page and fill our our New Patient Registration form and bring it with you to your first visit. 

Services that we offer in office to meet your needs

Our goal is to meet the needs of our patients first. We have trained medical assistants that are phlebotomy trained and able to take blood in office. Our office and staff are CLIA Certified which allows us to do certain laboratory testing while you wait.  If you are in need of testing that we are unable to provide in office, we make every effort to set up your appointment when it is convenient for you at the most accessible facility available.

Insurance and Payment

We accept several types of Healthcare Insurances including Medicare.  Contact our office at (513) 347-3302 for a complete list.  The patient is responsible for any insurance co-payments and any non-covered office charges at the time services are rendered.  For your convenience payments can be made by cash, check, and all major credit cards.  Receipts are available for use in obtaining reimbursement from your insurance company.  If there is a change in your insurance coverage at any point, please be sure to notify our office.
